Audit Trail

The audit trail is the append-only record of governed events — submissions, reviews, approvals, and changes — that lets a decision be reconstructed and a change be attributed.

Why it matters

Defensibility requires a record. The audit trail is how an organisation shows who did what, when, and on what basis.

What you can do

  • Trace governed events linked to an output or decision
  • Attribute a change to an actor and a point in time

What you cannot infer

  • Protected internal computation from the event record
